How to send your Equinox letter

1. Copy the template below and replace every [bracketed] variable. Delete lines that do not apply.
2. Check your agreement for the required notice method (mail, in person or online) and the notice address.
3. Send by tracked or certified mail, or hand it in and ask for a signed, dated copy.
4. Save a copy with the date and any tracking number or reference.
5. Diarise a follow-up in 7 to 14 days and check your statement for the next two billing cycles.
